The name of this event is FIIT OKC Wellness Gala and it is put on by MDM3 Fitness LLC doing business as FIIT Fitness Studio. Herein this agreement the entity will be reference to as FIIT OKC Wellness Gala.
Payment terms:
Payment is due at time of registration unless you choose the payment option. If the payment option is chosen half of the payment will be taken upfront base on which ticket that has been chosen. The other half of the other payment will be take 1 month later on the same day of the follow month.
Ticket price for this event may change or is subject to increase within the last month prior to the event date. with Payments received after this term may incur an additional administration charge. For changes in the registration, the organizer retains the right to charge an administration fee. For changes/cancellations an administration charge of 10% of the registration fee is applicable. It is possible to change the participant information to allow a replacement to attend the event in your place up-to 24 hours prior to the event start. Switching tickets to different event dates or cities are not permitted.
Refunds & cancellation:
FIIT OKC Wellness Gala does not offer refunds accept on the proven bases of extreme circumstances such of death or accident injury of an immediate family member within a 20 day window of the event scheduled date. Please be advice that you will be asked to show proof of such a incident of circumstance in order to be refunded. All refunds are subject to a %30 cancelation fee. So, please be sure that you will absolutely be able to make this event before you purchase any tickets. Cost of this event is base of absolute number of attendance. Our staff, planner, and vendors are guaranteed to be paid for their service. Thank you for understanding.
In case of insufficient number of registrations the organizer retains the right to cancel or postpone the event.
WAIVER
Consent to the Use of Photographic Images and Video for future use: Registrations and attendance at, or participation in FIIT OKC WELLNESS GALA (FOWG) events constitutes an agreement by the registrant to FOWG use and distribution (both now and in the future) of the registrant or attendee’s image or voice in photographs, videotapes, electronic reproductions, social media and audiotapes.
Liability Waiver & Release
In consideration of being allowed to register for, and participate in, this event I hereby warrant and represent that I am the age of 18 or above and freely waive, release from liability, assume all risks, and covenant not to sue FIIT OKC WELLNESS GALA (FOWG) or its members, employees, board members, agents, or volunteers for any expense, loss, damage, personal injury, including loss of life, disability, property damage, or property theft or actions of any kind that I may hereafter suffer, sustain or be responsible for before during or after the Event unless said expense, loss, damage, personal injury, including loss of life, disability, property damage or property theft or actions of any kind is caused by the sole, gross negligence of FOWG. This Waiver and Releases is specifically binding upon my heirs and assigns and is knowingly given.
